During a transforaminal injection, a small-gauge blunt needle is inserted into the epidural space through the bony opening of the existing nerve root. The needle is smaller than that used in a traditional epidural procedure. The surgery is carried out with the patient on their stomach under fluoroscopic (real-time x-ray) supervision, which helps to avoid nerve root injury. To improve the fluoroscopic pictures and ensure that the needle is appropriately inserted, a radiopaque dye is injected. The glucocorticoid treatment can be delivered closer to the inflamed nerve root with this procedure than with the traditional interlaminar epidural route. Radiation exposure is kept to a bare minimum.
